What Skim Perubatan Madani Is

Skim Perubatan Madani (often written SPM) is a Malaysian government health initiative that allows eligible lower-income Malaysians to be seen for common outpatient conditions at participating private GP clinics, rather than only at a government facility.

It is administered by the Ministry of Health together with ProtectHealth Corporation, which runs the electronic verification system that participating clinics use.

Why the scheme exists

The intention is to shorten waiting times at government facilities and let people with everyday illnesses be seen close to home — a child with a fever at 8pm, a food-poisoning episode over the weekend, a cut that needs cleaning and dressing.

What it generally covers

The scheme is broadly aimed at acute, everyday outpatient conditions — fever, cough and flu, sore throat, diarrhoea and vomiting, headaches, minor wounds and dressings, sprains and muscle pain.

It is generally not intended to cover routine health screening, vaccinations, long-term chronic follow-up, or supplements taken without a consultation. Scope and allocation are set by the Ministry of Health and can be revised, so the counter check on the day of your visit is the reliable answer for that visit.

The annual allocation

Each eligible category carries an annual allocation, drawn down visit by visit until it is used up, and the amount differs between the household, senior and single categories. Anything falling outside the allocation or the covered scope is handled as an ordinary clinic visit, explained at the counter before you are seen.

Who Is Generally Eligible

Here is the part that resolves most of the confusion: eligibility is linked to Sumbangan Tunai Rahmah (STR), and it is automatic. There is no separate Madani application form to fill in and no card to apply for.

If you are a Malaysian citizen who receives STR, you very likely already fall inside the scheme. The main recipient categories are:

  • Isi Rumah (household) — married couples and families receiving STR, including those with children
  • Warga Emas (seniors, 60 and above) — older Malaysians receiving STR, including those living alone
  • Bujang (single) — eligible single individuals of adult working age who receive STR

Common misunderstandings

Three beliefs stop people from checking at all:

  • “I have to apply first.” You do not. Eligibility follows your STR status.
  • “It only works at a government klinik kesihatan.” The opposite is true — the scheme exists precisely so that participating private GP clinics can see you.
  • “I need a card or a letter.” No card is issued. Your MyKad is the identifier.

How to Do a Madani Check With Your IC

There are four practical routes, and they answer slightly different questions.

1. Check your STR status first

Because Madani eligibility follows STR, your STR status is the upstream answer. It can be checked on the official Sumbangan Tunai Rahmah portal run by Lembaga Hasil Dalam Negeri, using your IC number. If your STR is approved and active, you are on the right side of the main criterion. If you have never applied for STR at all, that is the first gap to close — not the Madani side.

2. Check inside MySejahtera

MySejahtera carries a Madani medical scheme section for eligible users. Log in with the account tied to your own IC, find the Madani or health-scheme entry in the menu, and read the status shown. This is the quickest check to do yourself, and it works from home. If nothing appears, that does not automatically mean you are excluded — app sections vary by version and by whether your profile is verified. Treat a blank result as “unconfirmed” rather than “no”.

3. Check through the ProtectHealth channels

ProtectHealth Corporation administers the scheme’s verification system and publishes official public information on eligibility, together with the listing of participating clinics. This is the authoritative reference point — far more so than third-party lists on social media, which go stale quickly.

4. Let a participating clinic check it electronically

This is the most definitive route, and the one most people end up using. At a participating clinic, the front desk enters your IC into the scheme’s system and the result comes back within minutes — including whether allocation remains for the year. It is decisive because it checks what actually matters: your status today, in the system that authorises the visit.

What to Bring for the Check

Very little, in practice:

  • Your MyKad (IC) — the original document, not a photo. The number alone is not enough for the electronic check.
  • The IC of each family member being seen, if you are bringing children or elderly parents. Each person is verified individually against their own record.
  • A birth certificate or MyKid for young children who do not yet hold a MyKad.
  • A current medication list, or the medicine boxes themselves, if you take regular medication — for safe prescribing rather than for eligibility.

No appointment is needed at a walk-in GP clinic, and the eligibility check itself is done at registration, before you see the doctor.

If the Check Says You Are Not Eligible, or You Are Unsure

An unclear or negative result is common enough to be worth a sequence.

Step 1: rule out the obvious mismatches

Mis-keyed IC digits, a recently renewed MyKad, an STR application that has lapsed, or a household record filed under a spouse’s name all produce confusing results. Ask the counter to re-enter the number and confirm which name the record returns.

Step 2: separate “not eligible” from “allocation used up”

These are different answers. If your allocation has already been drawn down across earlier visits, you remain eligible — the benefit for that year is simply exhausted. The counter can tell you which you are looking at.

Step 3: check what else may cover the visit

Madani is one route among several. A visit may instead fall under PERKESO/SOCSO cover for a work-related injury or illness, or under an insurance or employer panel arrangement. Step 4: resolve the STR side separately

Eligibility is an administrative question. Some symptoms are not.

Please go straight to a hospital emergency department, rather than working through a coverage check first, if you or someone with you has:

  • Chest pain or chest tightness, especially with sweating, nausea or breathlessness
  • Sudden weakness or numbness on one side, facial droop, or slurred speech
  • Severe difficulty breathing
  • A fit or seizure, or a child who is drowsy, floppy or very difficult to rouse
  • Severe or persistent abdominal pain
  • Heavy bleeding that does not stop with firm pressure
  • A high fever in an infant under three months old
